IPAC Canada Long Term Care in Infection Prevention and Control Course

Please complete all information in the registration form. Have your Social Insurance Number (SIN) at hand, and your completed Tuition Payment Form ready to upload. Tuition fees are $2499 (tax exempt) for applications received by August 1, 2024. After that date, (and if space is still available) tuition fees are $2699. Tuition is payable in full with your registration.
Ensure you enter your given name (no nicknames) and your HOME address. When finished and your Tuition Payment Form has been uploaded, click 'Submit'.
You will receive an automatic reply to indicate your application was submitted. If you do NOT receive the auto-reply, please email admin@ipac-canada.org to inquire if your application was received. DO NOT submit a duplicate application unless we advise you your application was not received.
The deadline for applications is August 1, 2024. However, if the course fills to capacity before that date, registration will close. E-notifications regarding the course, materials, and instructions will come from the course coordinators' email, basicde@ipac-canada.org - add this as a 'safe sender' to your Address Book.
Course start date is September 1, 2024. In-person sessions in Toronto October 18th to 20th.
